Our Pavilion Family Medicine Online Patient Portal is now integrated with your electronic medical record. Please go to PFM.myupdox.com to register and create an account. Once your clinical summary has been sent, you will have access to your lab reports and other information from your medical record. You will also be able to communicate with us safely regarding non-emergent questions.

 

 

New Patient InformationNew Patients:

We are currently accepting new patients and would be happy to serve all of your health care needs.  New patients please download and print this New Patient Information Packet.  Please fill out the information required and bring or mail to Pavilion Family Medicine office prior to your first appointment. You will receive a confirmation and prompt reply as soon as a representative from our office reviews your information. 

The New Patient Information Packet is a twelve page form containing the following information vital for your medical records:

•  Authorization to Disclose Health Information
